Restore OPC welcomes the opportunity to partner with you in providing Orthotics, Prosthetics, or Cranial Romolding care for your patients. We make the process easy for you and your patients so they can get the excellent and compassionate care they need. We also respect the relationships you have with your patients and we are committed to open communication and follow-up after your patient is seen.


REFERRING YOUR PATIENT

  1. You provide a prescription, complete with diagnosis codes and notes.

  2. Patient contacts a local RESTORE OPC office to schedule an appointment.

  3. Our clinician evaluates your patient. Any relevant patient concern will be discussed during the consultation. It is also during this visit that we verify insurance benefits and discuss these with the patient.

  4. Our clinician / fabrication center fabricates the device needed by the patient.

  5. Our orthotist or prosthetist fits device to the patient.

  6. Patient is scheduled for a follow-up visit and we report outcomes back to physician.


OUR GUIDELINES

  • PATIENT MUST BE SEEN FOR AN OFFICIAL EVALUATION BY AN RNP, PA, OR MD

 

  • WE WILL NEED AN OFFICIAL SIGNED AND DATED OFFICE VISIT NOTE FROM THIS EVALUATION STATING THE TYPE OF DEVICE NEEDED

 

  • THE NOTE MUST STATE EXACTLY WHY THE DEVICE IS NEEDED

    i.e.; “Patient is indicated for one left Ankle Foot Orthotic (AFO) due to ankle instability.” “Patient is indicated for one right below knee prosthesis (BKPx) due to traumatic amputation of the right leg.”

 

  • ALWAYS INDICATE THE K LEVEL OF THE PATIENT

    (K0-bed ridden; K1-transfer only; K2-single cadence walker; K3-varied cadence walker; K4-runs and jumps with ease.)

 

  • THE DIAGNOSIS MUST FIT THE DEVICE NEEDED.

    i.e.; KNEE BRACE FOR KNEE INSTABILITY, KNEE BRACE FOR OSTEOARTHRITIS OF THE KNEE.

 

  • IT IS NOT COMPLIANT TO USE A GENERAL DIAGNOSIS

    i.e. Cerebral Palsy, Unstable Gait, Multiple Sclerosis, Poor Balance.

 

  • WE WILL NEED AN Rx SIGNED AND DATED THE DAY OF THE EVALUATION (OR AFTER)

 

  • AN Rx IS NOT COMPLIANT IF WRITTEN BEFORE THE DATE OF THE OFFICE VISIT.
